Upper and Lower Blepharoplasty

Blepharoplasty can be done on the upper lids, lower lids, or all four eyelids. It may remove skin, fat, or both to improve eyelid appearance.Functional upper lid blepharoplasty removes excessive skin that blocks vision, pushes against the lashes, or lowers the eyelid over the pupil. This is often covered by insurance plans including Medicare.

Indications for blepharoplasty

  • Excessive skin, called dermatochalasis, or fat bulging in the upper or lower lids can be corrected with cosmetic blepharoplasty.
  • Functional upper lid blepharoplasty may be covered by insurance, including Medicare, when upper lid skin touches the lashes and interferes with vision.

Why blepharoplasty as opposed to lasers or brow lifting?

  • Laser Lift can tighten some upper or lower lid skin, but surgery is generally required to remove larger amounts of skin. Lasers cannot remove eyelid fat.
  • Brow lifting improves low brows only. It does not remove eyelid skin and is not covered by insurance.
  • Blepharoplasty is the only method to remove eyelid fat and may be covered by insurance when done for functional upper lid concerns.

What to expect after a blepharoplasty

  • Bruising and swelling vary by patient, blood thinner use, genetics, and extent of fat removal. Most bruising is gone within 7-10 days.
  • Head elevation and ice can reduce bruising and swelling, especially during the first 36 hours.
  • Sleeping on the back is encouraged for a few weeks to avoid pushing or pulling on the lids after surgery.
  • Pain is usually minimal. Most patients report almost no pain if they take Tylenol routinely for 2-3 days and use ice.
  • Upper lid sutures are removed in 10 days. Depending on healing, lower lid sutures may be removed in 6-7 days.
  • Wearing eye makeup is variable, but most patients can wear mascara after suture removal and eyeliner in 2-3 weeks.
